Legal resident of the United States
Currently have NO healthcare insurance (including Medicaid or Medicare) insurance coverage
Current commercial insurance coverage, but doesn't cover my medication
Paying full,out-of-pocket price for prescription medication
Meet certain income eligibility requirements
